Cardiac Rehab+ Activ8rlives Self-Care education & Loan Equipment
What is Self-Care? If you attend One Leisure Right Start or the Cardiac Rehabilitation programme, you are already doing Self-Care. Self-Care is a series of simple actions which help you stay well, especially if we have one or more long-term health conditions. Being active is the best form of Self-Care in action. Eating a balanced diet to maintain a healthy weight is also an element of Self-Care. Simple health monitors can also help us track our vital signs and symptoms.
Knowing when to act and acting early are key to keeping well and can be used to avoid hospitalisation in the future. Over 8 weeks and for just 30 minutes after your Cardiac Rehabilitation exercise class at One Leisure, you will be taught by Activ8rlives how to use their latest Self-Care technology to record your own vital signs at home.
What do I get?
You will be loaned for 6 months: Blood Pressure Monitor, BuddyBand2 Waterproof Activity & Sleep Monitor, Smart Scales, Pulse Oximeter and Peak Flow Monitor. You will need to bring a Bluetooth 4.0-enabled Smart phone or Tablet (iOS 10 or Android 5.0 or later) which can connect to WiFi. Full instruction will be provided.
What will it cost me?
Just the normal session price (£3.50 or £31 for 10 classes) and for 6 months (extended by consultation with you), you will be loaned the medical monitors by Huntingdonshire District Council. FREE—NOTHING TO PAY.
Over 8 weeks, you will be instructed how to use the medical monitors and Activ8rlives App, how to connect them to your Smart phone or Tablet, what the information means and how to share data with your Doctors.
At the end of the 6 month loan programme (or at anytime during), you will be able to purchase these at a 20% discount, if you want to keep the equipment but cannot attend classes. If you don’t want to continue to self-monitor, simply return the equipment. NOTHING TO PAY: we only ask two things in return.
(1) you keep attending the CR exercise classes 3 weeks out of 4, and
(2) you use the monitors 4 days a week.
